Specific Admission Requirements

 Phlebotomy

• A copy of a picture ID, or Driver’s License
• High School Diploma or GED
• TB Test:  Must have before clinical.  If positive, must have a chest x-ray done at the health department or at your doctor’s office.

Certified Nurse Aide I

• A copy of a picture ID, or Driver’s License
• High School Diploma or GED
• TB Test:  Must have before clinical.  If positive, must have a chest x-ray done at the health department or at your doctor’s office.

 PCA (Personal Care Aide):

• A copy of a picture ID, or Driver’s License
• High School Diploma or GED (Not Mandatory) 
• TB Test: Must have before clinical.  If positive, must have a chest x-ray done at the health department or at the doctor’s office
